Comments & Experience Form

• Online – register and pay for your programs.  Search for your favourite program by keyword, age, day, time, location and more.
• In-Person – register at the Fleming Centre (5020 Serena Dr., Beamsville), Lincoln Museum and Cultural Centre (3800 Main St., Jordan) or Town Hall (4800 South Service Rd. Beamsville).
• Need assistance registering for a program?  Reach out to our customer service team and they would be pleased to assist.  A member of our team can be reached at 905-563-2799 ex 319.

• New registrations require a personal account with the Town of Lincoln. Your PlayLincoln account can be initialized in person at a Town facility or online.
• To create an account click on the “Sign Up” link on the login box.
• Complete all the fields on the pop-up box and click on the Submit button.
• You will receive an email from communication@perfectmind.com with a password.
• Click on the link in the email and enter in the username and password provided in the email.
• From here you can reset your password and update your account with additional family members, important info and add financial information, view schedules, transactions and activity outcomes under “My Profile”.

All program participants are required to sign an annual participation waiver as a part of the registration process.   Participant Waivers can be accessed online via your PlayLincoln account or through this link.  Paper copies are also available in Town facilities.

Often our popular programs will fill up fast. Avoid the frustration of missing out on a great program and add your name to the waitlist. If a spot becomes available, waitlisted participants will be added to the class in sequential order. If there are enough waitlisted participants, often a new program opportunity will be added.

• Stay up to date and receive the most current information on recreation and culture program offerings and events by signing up for our monthly newsletters:
  • Subscribe to Play Lincoln (recreation)
  • Subscribe to Lincoln Museum (culture)

• Refunds requested less than 5 business days prior to the start of the program, will be subject to a 50% cancellation fee per program/per participant
• A full refund or credit is given if a program is cancelled by the Town of Lincoln
• If the recreation program has started, refunds will be issued on a pro-rated basis
• For camps and swimming lessons no refunds will be issued once the program has started.

• All facility, room and/or park permit rental fees must be paid at time of booking to reserve a space.
• A non-refundable $25.00 administration fee will be charged for all cancelled permits.
• Permit Holders are required to request in writing of any cancellation a minimum of five (5) business days prior to the event. A refund minus the administrative fee will be issued.
• Should the Permit Holder cancel the event and not notify in writing prior to a minimum of five (5) business days prior to the event, the Permit Holder will be held responsible for full payment of the permit.

To withdraw yourself or family member from a drop in activities such as public skating or leisure swim follow these steps:

• Login into your PlayLincoln Account
• Go to “My Info” and click on the individual that you wish to withdraw from the program
• Click on Schedules
• Choose the program you want to withdraw and click actions
• Choose Withdraw from the action menu
• You can choose to have your refund as a credit on account which can be used for a future purchase or refund to the same method of payment.
• Complete the progress by choosing a reason.
• You can choose to receive an email or print confirmation

A support person is an individual hired or chosen to accompany a person with a disability in order to help with communication, mobility, personal care, medical needs or access to goods or services. A support person could be a paid personal support worker, a volunteer, a friend or a family member.  If you are a person with a disability that requires the accompaniment of a support person to participate in programs and activities, please identify this at time of registration.
